Abstract

Aim. In assuming that a portion of the adipose tissue within the neck region acts as an ectopic compartment, we intend to determine the expression of adiponectin and omentin-1 within the subcutaneous and paracarotid adipose tissue of the neck, analyze the relationship of serum adipocytokine levels and evaluate the metabolic risk factors in our examinees. ----- Materials and methods. 60 patients who underwent neck surgery were included in the study. Routine laboratory tests, anthropometric measurements and measurements of the carotid intima-media thickness were performed. Serum concentrations of total adiponectin and omentin-1 were determined. Expression of the adiponectin and omentin-1 genes in subcutaneous and paracarotid neck adipose tissue was analyzed. ----- Results. We found a statistically significant difference between the expression of adiponectin in subcutaneous and in paracarotid adipose tissue. Circulating levels of adiponectin were higher in subjects without metabolic syndrome and positively correlated with the level of HDL-cholesterol and negatively correlated with the neck and waist circumferences, glucose and insulin levels and the HOMA index. We did not find a statistically significant difference in the expression of omentin-1 between the subcutaneous and the paracarotid compartments nor did we did find a correlation between the omentin-1 expression in the tissues using our analyzed variables. The serum level of omentin-1 has a statistically significant negative correlation with body weight, BMI and triglyceride levels. We found a statistically significant positive correlation of neck circumference with body mass, waist circumference, BMI, insulin level and HOMA index. ----- Conclusion. These results support the fact that, although relatively small, the adipose tissue of the neck is positioned as a significant and specific depot with a possible parachrine and systemic influence and as a depot that is an independent predictor of metabolic risk beyond the existing standards of measurement.

Abstract in Croatian

Cilj. Pretpostavljajući da bar dio masnog tkiva vrata djeluje kao ektopični odjeljak, namjera nam je bila odrediti izražaj adiponektina i omentina u potkožnom i parakarotidnom masnom tkivu vrata te njihov odnos s razinom tih adipokina u serumu i čimbenicima metaboličkog rizika u naših ispitanika. ----- Metode i ispitanici. U istraživanje je uključeno 60 ispitanika koji su podvrgnuti operativnom zahvatu na vratu. Učinjene su rutinske laboratorijske pretrage, antropometrijska mjerenja, ultrazvučno mjerenje CIMT-a i analiza sastava tijela. Određena je serumska koncentracija ukupnog adiponektina i omentina-1 te izražaj gena za adiponektin i omentin-1 u potkožnom i parakarotidnom masnom tkivu vrata. ----- Rezultati. Našli smo statistički značajnu razliku između izražaja adiponektina u potkoţnom i u parakarotidnom masnom tkivu. Cirkulirajuća koncentracija adiponektina je viša kod ispitanika bez metaboličkog sindroma, pozitivno korelira sa HDL-kolesterolom, negativno s opsegom vrata i struka, inzulinom i HOMA indeksom. Nismo našli statistički značajnu razliku izražaja omentina-1 između potkožnog i parakarotidnog odjeljka ni značajne korelacije između izražaja omentina-1 u tkivima s analiziranim varijablama. Serumska razina omentina-1 negativno korelira s tjelesnom masom, ITM i trigliceridima. Našli smo statistički značajnu pozitivnu korelaciju opsega vrata s tjelesnom masom, opsegom struka, ITM, inzulinom i HOMA indeksom. ----- Zaključak. Ovi rezultati govore u prilog činjenici da je masno tkivo vrata specifično skladište sa mogućim parakrinim i sustavnim utjecajem, ali i skladište koje je nezavisni prediktor metaboličkog rizika izvan postojećih standardnih mjerila.
