Abstract
Background: Dipeptidyl peptidase-4 (DPP-44) is a serine protease recently linked to insulin resistance (IR), metabolic syndrome (MS) and microvascular complications in diabetes. We investigated the relationship between serum DPP-4 activity, IR, MS and microvascular complications in persons with type 1 diabetes (PWT1D). Methods: The study comprised 43 PWT1D aged >18 and <65 years. Insulin sensitivity was calculated using the equation for estimated glucose disposal rate (eGDR) which is in inverse correlation with IR. DPP-4 activity was determined spectrophotometrically as a rate of cleavage of 7-Amino-4-Methyl Coumarin (AMC) from H-Gly-Pro-AMC. Results: Participants were divided according to median DPP-4 serum activity (32.52 U/L) into two groups. Those with higher serum DPP-4 activity had lower eGDR (p<0.001), estimated glomerular filtration rate (eGFR) (p=0.021) while higher retinopathy (p<0.001) and autonomic neuropathy presence (p=0.001). The linear regression showed that eGDR decreases for 0.218 mgkg-1min-1 and eGFR for 1,49 ml/min/1,73m2 by each increase of DPP- 4 activity of 1 U/L after appropriate adjustments. Multivariate binary regression found that DPP-4 activity is associated with MS (1.392 (1.206-1.941), retinopathy (1.388 (1.089-1.645)) and autonomic neuropathy (1.813 (1.143-2.875)) presence. Conclusion: Serum DPP-4 activity is associated with IR, MS and the presence of microvascular complications in PWT1D. Its role in the pathophysiology of IR and microvascular complications merits to be further investigated.

Cilj: Dipeptidil peptidaza-4 (DPP-4) je ubikvitarna serinska proteaza odnedavno poznata i kao adipocitokin povezan s inzulinskom rezistencijom (IR) koja predstavlja okosnicu metaboličkog sindroma (MS) te čimbenik rizika za razvoj mikrovaskularnih komplikacija šećerne bolesti (ŠB). Cilj ovog istraživanja bio je ispitati povezanost aktivnosti DPP-4 u serumu s inzulinskom osjetljivošću, MS te mikrovaskularnim komplikacijama ŠB: retinopatijom, dijabetičkom bolesti bubrega te neuropatijom u osoba sa tipom 1 ŠB. - Metode i ispitanici: U istraživanje su uključene 43 osobe sa tipom 1 ŠB dobi između 18 i 65 godina. Inzulinska osjetljivost procijenjena je pomoću stope raspoloživosti glukoze (eGDR) koja obrnuto korelira sa stupnjem IR. Aktivnost DPP-4 u serumu određena je spektroflurimetrijski brzinom razgradnje H-Gly-Pro-amino-metil kumarina (AMC) u 7- Amino-4-AMC. Rezultati: Ispitanici su podijeljeni u dvije skupine prema srednjoj vrijednosti DPP-4 aktivnosti u serumu (32,52 U/L). Skupina ispitanika sa DPP-4 aktivnošću >32,52 U/L imala je nižu eGDR (p<0.001), nižu procijenjenu glomerularnu filtraciju (eGFR) (p=0.021) te veću učestalost neproliferativne i proliferativne retinopatije (p<0.001) i autonomne neuropatije (p=0.001). Linerna regresijka analiza pokazala je kako za svaki jedinični porast aktivnosti DPP-4 u serumu eGDR opada za 0,218 mgkg-1min-1, a eGFR za 1,49 ml/min/1,73m2 nakon prilagodbe za dob, spol, trajanje ŠB, HbA1c te upotrebu statina i antihipertenziva. Multivarijatna multinomilanla raščlamba pokazala je da je aktivnost DPP-4 u serumu povezana sa MS (1,392 (1,206-1,941) te učestalošću retinopatije (1,388 (1,089-1,645)) i autonomne neuropatije (1,813 (1,143-2,875)). Zaključak: Viša aktivnost DPP-4 u serumu povezana je s IR, nižom eGFR te većom učestalošću retinopatije I autonomne neuropatije u osoba sa ŠB tipa 1. Njen prediktivni značaj u patofiziologiji IR I razvoja mikrovaskularnih komplikacija potrebno je razjasniti daljnjim istraživanjima.
