Abstract

Spontaneous abortion is the most common adverse pregnancy outcome, and stress has been suggested to be important factor. We hypothesized that those female pregnant women with previous spontaneous abortion (n-100) will have higher anxiety and depression than female pregnant women without previous spontaneous abortion (n-100). Age and other sociodemographic parameters did not differ significantly between the groups of participants. Anxiety and depression levels were assessed with the Beck Depression Inventory and the Beck Anxiety Inventory. There were no significant differences in the scores on Beck Depression Inventory between groups of participants at the week 10 of gestation-1st assessment, week 20 of gestation-2nd assessment and week 30 of gestation-3rd assessment. Contrary of these results, pregnant women with history of spontaneous abortion have had statistically significant higher anxiety score than pregnant women without history of spontaneous abortion, at all of three assessments. Negative correlations between months passed form the last spontaneous abortion and anxiety on both assessment, emphasize the role of psychological support for the women who have experienced spontaneous abortion. The results on Coping Inventory for Stressful Situations showed that pregnant women with history of spontaneous abortion have had higher levels of avoidant mechanisms and emotional response to stress and lower level of cognitive response to stress than pregnant women without history of spontaneous abortion. Holmes-Rahe life stress inventory showed that pregnant women with history of spontaneous abortion have had higher levels of prior stressors than pregnant women without history of spontaneous abortion.

Spontani pobačaji smatraju se jednim od glavnih nepovoljnih ishoda trudoća, a uloga stresa pri tome se smatra bitnom. Hipoteza našeg istraživanja je da trudnice s anamnezom prethodnih spontanih pobačaja (n-100) imaju višu razinu depresije i anksioznosti u odnosu na trudnice bez spontanog pobačaja u anamnezi (n-100). Skupine se nisu razlikovale po dobi i ostalim socio-demografskim parametrima. Razina simptoma anksioznosti i depresije mjerila se Beckovima skalama za anksioznost i depresiju. Po pitanju izraženosti simptoma depresije, skupine se nisu razlikovale niti u jednom od tri trimestra trudnoće (mjereno u 10., 20. i 30. tjednu trudnoće). Anksioznost se pokazala razlikovnim faktorom, tako da su trudnice s anamnezom prethodnih spontanih pobačaja, kroz sva tri trimestra trudnoće imale značajno višu razinu anksioznosti u odnosu na trudnice bez spontanog pobačaja u anamnezi. Ustanovljena je negativna korelacija između mjeseci proteklih od spontanog pobačaja i anksioznosti tijekom trudnoće. Razlike u načinu suočavanja sa stresorima (Coping Inventory for Stressful Situations) između ispitanica koje imaju spontani pobačaj u anamnezi i ispitanica bez spontanog pobačaja u anamnezi pokazuju da je kognitivni odgovor na stresor značajno češći u skupini ispitanica bez spontanog pobačaja u anamnezi. Paralelno, emocionalni odgovor na stresor te izbjegavajuće ponašanje značajno su češći u skupini ispitanica koje imaju spontani pobačaj u anamnezi. Postoje i razlike u razini procjene prethodnih životnih stresora (The Holmes-Rahe Life Stress Inventory), koje ukazuju da ispitanice koje imaju spontani pobačaj u anamnezi, imaju značajno nižu učestalost niske razine prethodnih životnih stresora, u odnosu na ispitanice bez spontanog pobačaja u anamnezi. Isto tako vidljivo je da ispitanice koje imaju spontani pobačaj u anamnezi, imaju značajno višu učestalost umjerene kao i visoke razine prethodnih životnih stresora, u odnosu na ispitanice bez spontanog pobačaja u anamnezi.
