Abstract

AIM: The aim of this study was to examine the effect of dilatation of atherosclerotic renal artery stenosis (RAS) by endovascular intervention on augmentation index (AIx) and aortic pulse wave velocity (PWVao). MATERIALS AND METHODS: Thirty-one patients, 15 (48,4%) male and 16 (51,6%) female, average age 64,3 ± 10,3 years, with diagnosis of hemodynamically significant atherosclerotic RAS were included in this study. All patients had digital subtraction angiography (DSA) of renal arteries performed before the endovascular intervention. In all patients AIx and PWVao were noninvasively measured, before and 6 months after the endovascular treatment, using Arteriograph (TensioMedTM, Budapest, Hungary). RESULTS: A statistically significant decrease of AIx (-12,3 ± 12,2%, P <0,001) and PWVao (-1,1 ± 2,0 m / s, P = 0,005) was registered in the 6-month control period after the endovascular intervention, which proved a positive correlation between interventional procedure and a statistically significant decrease of AIx (P <0,001) and PWVao (P = 0,002). CONCLUSIONS: Endovascular renal intervention, together with antihypertensive medication, in hemodynamically significant atherosclerotic RAS of 60% or more, induces changes in AIx and PWVao value, leading to their significant decrease in the control period of 6 months after the procedure. Further investigations are required to include a larger number of patients and longer follow-up period in order to further evaluate and validate obtained results.

CILJ: Cilj istraživanja je bio ispitati utjecaj proširivanja aterosklerotskoga suženja bubrežne arterije (SBA) endovaskularnom intervencijom na augmentacijski indeks (AIx) i aortnu brzinu pulsnog vala (PWVao). MATERIJALI I METODE: U istraživanje smo uključili 31 bolesnika, 15 (48,4%) muških i 16 ženskih (51,6%), prosječne životne dobi 64,3 ± 10,3 godine, s dijagnozom hemodinamski značajnog aterosklerotskoga SBA. Svim bolesnicima prije endovaskularne intervencije učinjena je digitalna suptrakcijska angiografija (DSA) bubrežnih arterija. Kod svih ispitanika neinvazivno su izmjereni Aix i PWVao uređajem Arteriograph (TensioMedTM, Budimpešta, Mađarska) prije i 6 mjeseci nakon endovaskularne intervencije. REZULTATI: Registrirali smo statistički značajan pad AIx (-12,3 ± 12,2%; P<0,001) i PWVao (-1,1 ± 2,0 m/s; P=0,005) u kontrolnom periodu od 6 mjeseci nakon endovaskularne intervencije i dokazali smo pozitivnu povezanost intervencijskog zahvata na statistički značajan pad AIx (P<0,001) i PWVao (P=0,002). ZAKLJUČCI: Endovaskularna bubrežna intervencija uz primjenu antihipertenzivnih lijekova kod hemodinamski značajnog aterosklerotskoga SBA od 60% i više dovodi do promjena vrijednosti AIx i PWVao, uzrokujući njihov značajan pad u kontrolnom periodu od 6 mjeseci nakon provedenog zahvata. Potrebna su daljnja ispitivanja koja će uključiti veći broj ispitanika i duži vremenski period praćenja kako bi se moglo dodatno vrednovati i provjeriti dobivene rezultate.
