Abstract

The research objective was to estimate changes in the hypothalamic „set points“during treatment of the schizophrenic disorder. The research include 80 male, acute psychotic schizophrenic patients was hospitalised on the Psychiatric Clinic Osijek between March 2004. And April 2007.. All patients assigned Informed Consent for participating in the Study. Diagnosis of the schizophrenic disorder was based upon ICD-X criteria. Patient was treated with riperidone in therapeutic doses. First day of the investigation we measured: pulse rate, blood systolic and diastolic pressure, eye blinking rate, respiratory rate, body weight and high, blood glucose concentration, sodium and potassium concentrations, FSH, LH, TSH, HGH, ACTH, prolactine. We used PANSS rating scale for calculating intensity of disorder and group of symptoms. After 21 days we repeated measurements. According to results on the PANSS we divided patients in two basic groups: cured and no cured group. We compared these two groups according to changes between two measurements. Before starting treatment with antipsychotic therapy 2,5% patients had hyponatremia, 23,75% hyperglycemias, 7,5% had TSH and FSH elevation, 11,25% had LH elevation. 38,75% patients had prolactin concentration higher than normal range. There is no statistically significant changes in the regulation of the blood glucose level, systolic and diastolic blood pressure, pulse rate, eye blinking rate, potassium, FSH, LH, TSH, HGH blood concentrations. Changes in the sodium ( up regulation ), ACTH ( up regulation ), body weight ( up regulation ) and respiratory rate ( down regulation ) are statistically significant and may represent successfulness of the treatment. Hyperprolactinemia was not connected with successfulness of the treatment or changes in the group of the symptoms. It is pure side effect.

Istraživanjem smo pokušali procijeniti promjene regulacijskih mehanizama hipotalamusa tijekom liječenja shizofrenije. Istraživanjem smo obuhvatili 80 akutno psihotičnih shizofrenih bolesnika koji su hospitaliziranu u Psihijatrijskoj klinici u Osijeku u razdoblju od ožujka 2004. Godine do travnja 2007. Godine. Svi bolesnici su potpisali informirani pristanak za sudjelovanje u istraživanju. Dijagnoza shizofrenog poremećaja postavljena je prema MKB 10 dijagnostičkim kriterijima. Svi bolesnici su liječeni risperidonom u terapijskim dozama. Prvog dana istraživanja učinjena su mjerenja: puls, sistolički i dijastolički krvni tlak, broj udisaja, broj treptaja očnih vjeđa, visina, tjelesna težina, GUK, Na+, K+, FSH, LH, TSH, HGH, ACTH, prolaktin. Intenzitet bolesti i intenzitet pojedinih skupina simptoma procijenjen je ocjenskom ljestvicom PANSS. Nakon 21 dan mjerenja i ocijenska ljestvica PANSS su ponovljeni. Na osnovu rezultata ocjenske ljestvice PANSS bolesnici su podijeljeni u dvije skupine: oporavljeni i neoporavljeni. Ove dvije skupine su uspoređene s obzirom na razlike između mjerenja. Prije početka liječenja 2,5% bolesnika ima hiponatremiju, 5% hipokalemiju, 23,75% hiperglikemiju, 7,5 % povišene vrijednosti TSH, 7,5% povišene vrijednosti FSH, 11,25% povišene vrijednosti LH, 38,75% povišene vrijednosti prolaktina što ukazuje na primarni poremećaj u regulacijskim mehanizmima hipotalamusa. Tijekom liječenja nema značajnijih promjena u regulaciji sistoličkog i dijastoličkog krvnog tlaka, pulsa, broja treptaja očnih vjeđa, GUK, K+, FSH, LH, TSH, HGH. Tijekom liječenja dolazi do porasta koncentracije natrija, smanjenja broja udisaja, porasta tjelesne težine i ACTH što je povezano s uspjehom u liječenju. Tijekom liječenja značajno raste i koncentracija prolaktina što nije povezano s uspjehom u liječenju već se radi o čistoj nuspojavi.
