Abstract

The principal aim of this study was to investigate the correlation between serum and atherosclerotic plaque expression of resistin and clinical symptomatology as well as histological features of atherosclerotic plaque. The study included 78 patients with significant (>60 %) unilateral stenosis of carotid artery hospitalized due to carotid endarterectomy. Patients were divided into two groups based on clinical symptoms. Based on the histological features, the plaques were divided into groups I-VIII according AHA classification and into groups with unstable and stable plaque phenotype. Immunohistochemistry was performed with the primary antibody for human resistin (Resistin/RETN antibody, eBioscience), and the resistin expression was determined semi-quantitatively. The serum concentration of resistin was determined by the ELISA method. A confirmed hypothesis that expression of resistin is higher in unstable plaques indicates its association with the plaque destabilization process and suggests that resistin could potentially be used as one of the histological markers of plaque instability. Significant correlation of resistin with the symptoms of the patient also indicates potential utilization of resistin to stratify high-risk patients. Further research will be required to detect potential drugs whose purpose would be to reduce serum resistin values and consequently reduce cardiovascular risk.

Osnovni cilj ovog istraživanja bio je istražiti povezanost izraženosti rezistina u serumu i aterosklerotskom plaku karotidne arterije s kliničkom slikom i histološkim izgledom plaka. U istraživanje je uključeno 78 bolesnika sa značajnim (>60 %) unilateralnim suženjem karotidne arterije hospitaliziranih radi karotidne endarterektomije. Bolesnici su podijeljeni na dvije skupine: asimptomatski i simptomatski bolesnici. Na temelju histološke analize i karakteristika plakovi su prema svojim značajkama podijeljeni temljem AHA klasifikacije u skupine od I - VIII, odnosno na one s nestabilnim i stabilnim fenotipom. Imunohistokemija je učinjena s primarnim protutijelom za ljudski rezistin (Resistin/RETN antibody, eBioscience), a izraženost rezistina utvrđena je semikvantitativno. Serumska koncentracija rezistina određena je ELISA metodom. Potvrđena hipoteza kako je rezistin više izražen u karotidnim plakovima simptomatskih bolesnika i u histološki nestabilnim plakovima ukazuje na njegovu povezanost s procesom destabilizacije plaka. Značajna povezanost rezistina s histološkom slikom plaka i sa simptomatologijom upućuje na to kako bi se rezistin potencijalno mogao koristiti kao jedan od histoloških markera nestabilnosti plaka. Značajna povezanost rezistina sa simptomima bolesnika također ukazuje na potencijalnu iskoristivost rezistina u stratificiranju visokorizičnih bolesnika. S obzirom na navedeno, bit će potrebna daljnja istraživanja u svrhu otkrivanja potencijalnih lijekova čija bi svrha bila smanjivanje vrijednosti rezistina u serumu te posljedično smanjenje kardiovaskularnog rizika.
