Abstract

Hearing impairment and specific language disorder are two entities that seriously affect language acquisition ability in children and reduce their communication skills. These children require specific treatment and higher level of care. Their language abilities also strongly influence parent child interactions. The purpose of our study was to evaluate health related quality of life (HRQOL) in parents of hearing impaired children, and parents of children with speech difficulties (specific language disorder). The subjects of our study were 349 parents (182 mothers and 167 fathers) of preschool-age children with receptive expressive language disorder and 131 parents (71 mothers and 60 fathers) of children with severe hearing impairment. A control group was composed of 146 parents (82 mothers and 64 fathers) of healthy children of the same age. HRQOL was assessed using SF-36 questionnaire. In all groups of parents mothers scored poorer results compared to the fathers, but with large differences depending on the impairment of the child. In the control group, the results of mothers were significantly lower in only two (out of eight) health domains, in mothers of speech impaired children in three domains, and in six domains in mothers of hearing impaired children, indicating the biggest difference between them and their spouses. When compared to control group, both mothers and fathers of speech impaired children scored signficantly worse in five health dimensions. Fathers of hearing impaired children scored significantly worse than controls in three health dimensions. The lowest results, indicating the poorest HRQOL scored mothers of hearing impaired children, obtaining significantly lower results than control mothers in all of the health domains except for role emotional. The parents of pre-school speech and hearing imapired children experience poorer health related quality of life than parents of healthy children of the same age. Mothers of hearing impaired children are especially affected, demonstrating negative impact in almost all of the health domains.

Oštećenje sluha i specifični jezični poremećaj dva su entiteta koja ozbiljno narušavaju sposobnost usvajanja govora u djece i ograničavaju njihove komunikacijske mogućnosti. Ova djeca trebaju specifičan tretman i pojačanu razinu brige. Govorne sposobnosti značajno utječu i na interakciju dijete – roditelj. Cilj ovog rada je procijeniti kvalitetu života vezanu za zdravlje kod roditelja djece s oštećenjem sluha, te onih s govornim oštećenjem. Ispitanici u ovoj studiji bili su 349 roditelja (182 majke i 167 očeva) djece predškolske dobi sa specifičnim jezičnim poremećajem i 131 roditelj (71 majka i 60 očeva) djece s jakim slušnim oštećenjem. Kontrolnu skupinu sačinjavali su 146 roditelja (82 majke i 64 oca) zdrave djece iste dobi. Kvaliteta života procijenjena je upotrebom SF-36 upitnika. U svim skupinama ispitanika majke su imale slabiji rezultat u usporedbi s očevima, ali uz velike razlike u ovisnosti o vrsti teškoća u djeteta. U kontrolnoj skupini rezultati za majke bili su značajno različiti od onih kod očeva u samo dvije (od osam) dimenzija zdravlja, u majki djece s govornim oštećenjem u tri dimenzije, a u majki djece sa slušnim oštećenjem u šest dimenzija, ukazujući na najveću razliku između njih i njihovih muških partnera. U usporedbi s kontrolnom skupinom, i majke i očevi djece s govornim oštećenjem imali su značajno slabiji rezultat od kontrola u pet dimenzija zdravlja. Očevi djece oštećena sluha su bili značajno lošiji od kontrole u tri dimenzija. Najniži rezultat, koji ukazuje na najslabiju kvalitetu života vezanu za zdravlje imale su majke djece oštećena sluha koje su postigle značajno slabije rezultate od kontrolnih majki u svim ispitanim dimenzijama zdravlja osim „ograničenja radi emocionalnih smetnji“. Roditelji predškolske djece s oštećenjem sluha i govora imaju slabiju kvalitetu života vezanu za zdravlje, nego roditelji zdrave djece iste dobi. Naročito su pogođene majke djece oštećena sluha koje pokazuju negativan učinak djetetovih smetnji na gotovo sve dimenzije zdravlja.
