Abstract

INTRODUCTION: Previous research about lenticulostriatal vasculopathy (LSV) on intracranial ultrasonography in newborns in the diagnosis of congenital cytomegalovirus infection are contradictory. ----- OBJECTIVES: To determine the diagnostic value of brain ultrasound markers of congenital CMV infection in newborns and infants with developmental disorders with LSV as a leading marker. ----- SUBJECTS: The study included 234 patients with neurodevelopmental disorders and LSV, divided into two groups depending on the confirmed or excluded congenital CMV infection. With LSV as a leading, additional markers were recorded as follows: periventricular focal hyperechogenicity (PFE), paraventricular cysts, corpus callosum abnormalities (CC), ventriculomegalia (LV), periventricular diffuse hyperechogenicity (PVE), subependimal cysts, abnormal choroid plexus, wide subarachnoid spaces (VSP), abnormalities of gyri and sulci, hypoplasia of the cerebellum and the Doppler verified presence of flow within LSV. ----- RESULTS: Patients with congenital CMV infection with LSV have more of associated markers in intracranial ultrasonography. Observation of each marker in association with LSV showed statistically significant difference in the frequency of PFE, CC, PVE, VSP and LV in patients with congenital CMV infection. ----- CONCLUSION: The results of this study showed that despite the LSV is a nonspecific feature, in infants with developmental disorders iz may serve as a sign for screening for congenital CMV infection. As the number of markers increases the likelihood of congenital CMV infection rises. There is no specific pattern, but some of the markers show a significant difference in the frequency.

UVOD: Dosadašnja istraživanja značenja nalaza lentikulostrijatalne vaskulopatije (LSV) na intrakranijskoj ultrasonografiji u novorođenčadi u dijagnozi kongenitalne citomegalovirus infekcije su proturječna. ----- CILJEVI: Ustanoviti dijagnostičku vrijednost ultrazvučnog nalaza biljega kongenitalne CMV infekcije u populaciji novorođenčadi i dojenčadi sa neurorazvojnim odstupanjima s LSV-om kao vodećim biljegom. ----- ISPITANICI: U istraživanje je uključeno 234 pacijenta s neurorazvojnim odstupanjem i s ultrazvučnim nalazom LSV-a, te podijeljeno u dvije skupine ovisno o dokazanoj ili isključenoj kongenitalnoj CMV infekciji. Uz LSV kao vodeći, dodatno su evidentirani slijedeći biljezi: periventrikularne fokalne hiperehogenosti (PFE), paraventrikularna cista, abnormalnosti corpus callosuma (CC), ventrikulomegalija (LV), periventrikularna difuzna hieprehogenost (PVE), subependimalna cista, abnormalnosti plexus choroideusa, prošireni vanjski subarahnoidalni prostori (VSP), abnormalnosti u građi girusa i sulkusa, hipoplazija malog mozga te prisutnost protoka unutar LSV-a dopplerom. ----- REZULTATI: pacijenti s kongenitalnom CMV infekcijom na intrakranijskoj ultrasonografiji uz LSV imaju veći broj pridruženih biljega. Promatranjem svakog pojedinačnog biljega uz LSV nađena je statistička značajna razlika u učestalosti nalaza PFE, CC, PVE, VSP i LV u pacijenata s kongenitalnom CMV infekcijom. ----- ZAKLJUČAK: Rezultati ovog istraživanja su pokazali da unatoč tome što LSV predstavlja nespecifično obilježje može u dojenčadi s neurorazvojnim odstupanjima služiti kao znak za probir na kongenitalnu CMV infekciju. Kako raste broj biljega raste i vjerojatnost kongenitalne CMV infekcije, a specifičan uzorak ne postoji već neki od biljega pokazuju značajnu razliku u učestalosti.
