Abstract

The sentinel lymph node has a crucial role in the induction of an immunologic response as the first structure which drains lymph from the tumour. T regulatory lymphocytes are a CD4+CD25+ T-cell subset characterized by expression of transcription factor Foxp3. They serve as a principal mechanism to maintain self-tolerance and may be largely responsible for attenuating antitumor immune responses. The aim of this study was to asses the relationship between Foxp3+ lymphocytes and key prognostic factors for melanoma. An assumption was made by previous investigations that quantity of Foxp3+ lymphocytes is related to worse prognostic features. In this study Fox3+ lymphocyte counts were related to age, gender, localisation, thickness and ulceration of the primary tumor, presence of metastases and number of affected nodes. Additional comparison was done against imprint cellularity. The results indicate that only age and cellularity are significantly related to the number of Foxp3+ lymphocytes. Both variables inversely correlated to Foxp3+ count. It is suggested that younger people are more able to produce stronger immune response, with hyperplastic lymph nodes. Foxp3+ lymphocytes mediate immune reactions in order to stop them, to protect tissue from inflammation damage, or to serve tumor evasion.

Abstract in Croatian

Sentinel limfni čvor ima ključnu ulogu u aktivaciji imunološke reakcije jer drenira tumorske stanice i mjesto je gdje se tumorski produkti predočavaju naivnim limfocitima T. Tvore ga brojne stanice i topivi imuni medijatori koji usmjeruju imunološku reakciju. CD4+CD25+ limfociti T koji izražavaju transkripcijski faktor Foxp3 smatraju se regulacijskim limfocitima T koji služe kao glavni mehanizam periferne tolerancije i stoga mogu biti odgovorni za oslabljivanje imunološke reakcije protiv tumorskih antigena. U ovom radu uspoređen je broj Foxp3+ limfocita u citološkim otiscima sentinel limfnih čvorova s prognostičkim čimbenicima kod melanoma: dobi, spolom, lokalizacijom, debljinom i ulceracijom primarnog tumora, prisutnošću malignih stanica u limfnom čvoru, te brojem zahvaćenih limfnih čvorova. Osim toga, s brojem Foxp3+ limfocita usporedila se i celularnost otisaka kao pokazatelja funkcionalnog stanja čvora. Provedena analiza utvrdila je značajnu statističku povezanost većeg broja Foxp3+ limfocita s mlađom životnom dobi i celularnijim otiscima. Moguće objašnjenje za to je veća imunokompetencija u mlađih ljudi, samim time i stvaranje jake imunološke reakcije, pa i potrebe za njezinim obuzdavanjem.
