Abstract

One of the most severe forms of male infertility is non-obstructive azoospermia (NOA). NOA is frequently characterized by a heavy damage of seminiferous tubules that has already been described in the literature. Much less is known on the changes of Leydig cells. The hypothesis of this study is based on the assumption that Leydig cells in patients with NOA were significantly different (damaged) form the same type of cells in the control group. Therefore, the aim of the current survey was to investigate Leydig cells in two groups (control and the group of infertile men with NOA) by means of qualitative and quantitative histological methods and immunohistochemistry (the expression of testosterone in situ). In addition, blood levels of gonadotrophins and testosterone were determined. Results of qualitative histological analysis demonstrated a kind of “mosaic” picture of regular and irregular Leydig cells in the NOA cases. At average there were 35% irregular Leydig cells and their number correlates with testis volume and the status of spermatogenesis. Quantitative histological analysis indicated a significantly lower number of testosterone-producing cells (P<0,001). NOA patients also had significantly lower testis volume and the status of spermatogenesis when compared to controls. The results of the study pointed out that the patients with NOA could suffer from a deficit of androgens as well as a premature andropause.

Jedan od najtežih oblika neplodnosti u muškarca je neopstruktivna azoospermija (NOA). U pacijenata s NOA nerijetko dolazi do teških oštećenja sjemenskih kanalića koja su već proučena i opisana. Manje se zna o promjenama u intersticiju sjemenika, gdje su smještene Leydigove stanice. Hipoteza ovog rada je da se Leydigove stanice pacijenata s NOA značajno razlikuju od istih stanica kontrolne skupine. Cilj rada bio je proučiti Leydigove stanice u dvije skupine ispitanika (kontrolne i skupine neplodnih muškaraca s NOA) koristeći metode kvalitativne i kvantitativne histološke analize te imunohistokemije (izražaj testosterona in situ). Također su određene vrijednosti gonadotropina i testosterona u krvnom serumu pacijenata. Rezultati kvalitativne histološke analize pokazali su da u bioptičkim uzorcima sjemenika pacijenata s NOA postoji tzv. mozaična slika Leydigovih stanica: uz veći broj ovih stanica normalne morfologije utvrđeno je da je prosječno 35% intersticijskih stanica znatno poremećene građe. Brojnost promjenjenih Leydigovih stanica djelomično korelira sa stupnjem poremćaja spermatogeneze i volumenom sjemenika. Kvantitativnom histološkom analizom dokazano je da u bioptičkim uzorcima pacijenata s NOA postoji statistički značajno manji broj Leydigovih stanica koje proizvode testosteron. Volumen sjemenika i stupanj očuvanosti spermatogeneze značajno je niži u neplodnih pacijenata (P<0,001). Rezultati rada upućuju na mogućnost kako kratkotrajnog tako i dugotrajnog manjka androgena te preuranjene andropauze kod neplodnih muškaraca s NOA, što zahtjeva njihovo pažljivo praćenje i redovite kontrole.
